You may call after office hours for emergencies at (419) 473-9500 or toll free (800) 969-1215. Our phones are forwarded after office hours to the Academy of Medicine. The Academy of Medicine will contact Dr. Bowen or Dr. Woodard to call you, so please have a phone number available for them to reach you.
What should I bring to my appointment?
Please view the information for Visiting Our Office. You should bring with you the following: your insurance card, a list of your allergies and current medications, and any X-rays, MRIs, or other pertinent records necessary for the doctor.
Where are your offices located?
Dr. Bowen and Dr. Woodard see patients in Toledo at 4126 N. Holland-Sylvania Road, Suite 100. Be sure to visit our Location and Map for directions.
When will I be expected to pay my co-payment?
Co-payments are expected to be paid at the appointment.
