Visiting Our Office

When you visit us, please arrive shortly before your appointment time. You should bring with you the following: your insurance card or worker's compensation information, a list of your allergies and current medications, a copy of your referral (on your first visit if your insurance requires a referral), and any x-rays, MRIs, or other pertinent records necessary for the doctor.

On your first visit we will ask you to fill out some paperwork. You may do this in advance by viewing our Office Forms and printing the documents. Please be sure to bring these hard copies with you rather than email them to us.

In the evenings, our phones are forwarded to the Academy of Medicine. Please leave a message with your name and phone number to reach you at so that the doctor or physician assistant may reach you as soon as possible.

If you have a medical question, please call our office. During office hours, the doctor is seeing other patients and may not be able to take your call immediately, however please leave a message with the doctor's staff and we will return your call soon.

Co-payments are expected to be paid at the appointment.

Occassionally the doctor will be behind schedule because of emergency situations that may arise. We appreciate your patience and understanding in these unavoidable situations and we will make an attempt to inform you if a lengthy delay is expected.
